What Is a DOT Physical?

A DOT physical is a medical examination required by the Federal Motor Carrier Safety Administration (FMCSA) for anyone who operates a commercial motor vehicle (CMV) in interstate commerce. The exam ensures that drivers can safely perform their duties without putting themselves or the public at risk.

You need a DOT physical if you drive:

  • Vehicles weighing 10,001 pounds or more
  • Vehicles designed to transport 16+ passengers (including the driver)
  • Vehicles transporting hazardous materials
  • Any combination of the above in interstate commerce

What the Exam Covers

The DOT physical is comprehensive but straightforward. Here's what we'll check:

1

Vision Test

At least 20/40 in each eye (with or without correction) and at least 70° field of vision in each eye.

2

Hearing Test

Must perceive a "forced whisper" at 5 feet or less (with or without hearing aids).

3

Blood Pressure & Pulse

Blood pressure must be below 140/90 for a full 2-year certification. Higher readings may result in shorter certification periods.

4

Urinalysis

Tests for underlying medical conditions like diabetes. This is NOT a drug test.

5

Physical Examination

General health assessment including heart, lungs, abdomen, spine, and extremities.

6

Medical History Review

Discussion of current medications, past surgeries, and any chronic conditions.

How to Prepare for Your DOT Physical

📋 What to Bring

  • Valid photo ID (driver's license)
  • List of all current medications (including dosages)
  • Glasses or contacts if you wear them
  • Hearing aids if you use them
  • CPAP compliance report if you use one for sleep apnea
  • Letter from your doctor if you have diabetes, heart conditions, or other chronic issues
  • Previous DOT medical card (if renewing)

💡 Pro Tips for Success

Get good sleep the night before — fatigue can raise blood pressure. Avoid caffeine the morning of your exam. Eat a light meal but don't fast (low blood sugar can affect the exam). Wear comfortable clothing that's easy to move in.

Common Conditions and Certification

Having a medical condition doesn't automatically disqualify you. Many conditions can be managed while maintaining your CDL:

  • Diabetes: Certification possible if well-controlled. Insulin users may need additional documentation.
  • High blood pressure: Stage 1 hypertension (140-159/90-99) may get 1-year certification. Higher requires treatment first.
  • Sleep apnea: Certification possible with documented CPAP compliance.
  • Heart conditions: May require clearance from a cardiologist.
  • Vision/hearing issues: Can often be corrected with glasses, contacts, or hearing aids.

⚠️ Disqualifying Conditions

Certain conditions are disqualifying, including uncontrolled epilepsy, certain psychiatric disorders, and recent heart attacks or strokes. If you're unsure about your condition, talk to us before your exam — we can help you understand your options.

Certification Periods

Based on your health status, you may receive a medical certificate valid for:

  • 2 years: Standard certification for drivers in good health
  • 1 year: For conditions requiring annual monitoring (like Stage 1 hypertension)
  • 3-6 months: For conditions that need closer monitoring or time to demonstrate control

After Your Exam

If you pass, you'll receive your Medical Examiner's Certificate (the DOT medical card) the same day. The examiner will also submit your results to the FMCSA National Registry.

Important: You must provide a copy of your medical certificate to your state DMV within the timeframe required by your state to maintain your CDL medical certification status.
